Our Health Maintenance Navigator supports the work of our Clinician/Medical Assistant dyads by working with patients and their chart outside of the scheduled visit time. They provide support by facilitating patient engagement in chart preparation, updating completed health maintenance activities that have occurred outside of WHA/LEAP, and helping ensure engagement and completion of ordered health maintenance tests, treatments and consults. This position will assist by optimizing Wellness Visits and help facilitate a standardized approach to screening and resulting related to wellness care, and engage established patients in completing the provider's wellness care plan.

*This is a 40 hrs/wk position working Monday - Friday.


  • Serve as an effective communication link between patient and clinic staff by gathering information from patients.
  • Independently and proactively identify patients in the practice by running reports for specific diagnosis, preventive care needs, and orders not completed.
  • Communicates with patients directly to discuss preventative care needs. Refers patients with medical concerns to appropriate clinical staff.
  • Reviews provider schedules and patient charts to assist the care team in coordinating care for visits and identifying preventive healthcare needs.
  • Evaluate and analyze patient records, based on appropriate guidelines, for patients who need preventive services, diagnostics, and follow up.
  • Coordinate the scheduling of patients' preventive care needs based on their care plan and appropriate guidelines.
  • Keep health maintenance records updated for patient future scheduling needs
  • Coordinates and/or completes chart preparation for Wellness Exams and facilitates getting previous records or documentation needed for upcoming visits.
  • In coordination with Referral Navigator or other Patient Services staff, interacts with internal and external health care providers to ensure prompt and effective transitions of care.
  • Ensures all pap, mammogram and other health maintenance results are sent to the patient's primary care provider.
  • Works collaboratively with other members of the health care team in coordination of the patient's care.
  • Additional clinical duties and coordination of care as assigned.


  • High school diploma or GED required.
  • Graduate from an accredited medical assisting program.
  • Certified as a Medical Assistant through the AAMA, NCCT or AMT required
  • Knowledge of medical charting processes and procedures required. Prior experience with electronic medical records software preferred.
  • Proven effective verbal, listening and written communication skills required.
  • Knowledge of or ability to learn medical practice management scheduling software and time requirements for scheduling.
  • Demonstrated ability to work in a fast-paced environment and effectively work with others.
  • Knowledge of basic computer skills, including Word and Outlook.
  • Knowledge of medical terminology and ICD-9, CPT and HCPCS coding.
  • Must be able to pass a drug screen and background check.
  • Ability to demonstrate The Values Statement and The Patient Experience of WHA.
  • Ability to perform the essential functions of the job.
